FinalForms Launch for the 2025–2026 School Year

Each year, we evaluate our procedures in the Activities Office to find ways to improve. For the upcoming 2025–2026 school year, Pleasant Hill Middle & High School will no longer use PRIVIT. We’re excited to announce our transition to FinalForms, a more user-friendly and advanced platform for managing athletic and activity forms.
Why FinalForms?
FinalForms allows you to complete, sign, and manage all athletic/activity participation forms online. Once your account is created, it will notify our office, allowing us to upload your child's physical if you don’t have a paper copy. Parents/guardians must sign off on all required forms.
